To Receive your $50 rebate, the following must be submitted:

• The rebate form enclosed with your Perkl-Light™, filled out in its entirety.
• Original or a copy of your sales receipt, dated between 5/5/08 and 6/15/08.

Important:

• Your rebate submission must be signed, dated, and postmarked before 7/15/08.
• Your rebate will be mailed on or before 8/15/08.
• Rebate applies to retail sales to U.S. customers only.
• We suggest that you make a photocopy of your submitted materials for your records.
• Failure to follow these submission instructions may invalidate your rebate.

Any mail-in rebate form or check that is misdirected by the U.S. Postal Service is not the responsibility of Star Energetics LLC. Incorrect information, illegible or incomplete rebate forms will be considered invalid and ineligible for rebate and presumed to be the customers’ acts or omissions. Star Energetics LLC reserves the right to review and approve all offers and modify or withdraw this promotion at any time or for any reason. Rebate checks will be issued in U.S. dollars only. Rebate checks not cashed within six months of the date on the check will be considered void and cannot be reissued. This rebate has no cash value. Reproduction, purchase, sale or trade of this rebate form (other than to make a copy of the completed form for your records) is prohibited. Materials received become the property of Star Energetics LLC and cannot be returned. Offer valid in the United States for retail customers only.
