Information for New Retailers
If you are interested in carrying the Perkl-Light in your store, catalog, or clinic, please e-mail us at info@perkl-light.com, or phone us at 303-684-9810. We will need a copy of your reseller's (sales tax) license and some documentation that you are in an appropriate business before we can wholesale to you.
If you are located in Colorado, we also need the following form, which is required by the Colorado Department of Revenue. Please download it and fill it out so you are ready when you call. Thanks!
